成果简介NOVELTY - Invention relates to oncology, gynecology. A radical trachelectomy with transposition of the uterus, a course of chemoradiation therapy according to a radical program and a delayed reconstructive-plastic surgical stage with bringing down the uterus and forming a uterovaginal anastomosis are performed. At the same time, with the start of neoadjuvant polychemotherapy, gonadotropin-releasing analogs are prescribed. The course of taking the drug is completed immediately after the reconstructive surgery. Also, intraoperative radiation therapy (IORT) is performed at a dose of 10 Gy per vaginal stump during radical trachelectomy after fixation of the uterus in the paraumbilical region. Then, a course of combined radiation therapy (CRT) is carried out according to a radical program in a standard mode: external radiation therapy, TBD 46 Gy, intracavitary radiation therapy, TBD 50 Gy, a reconstructive-plastic surgical stage is performed on the parametric zone and lymph outflow pathways 3 weeks after the end of the course of CRT, during which the obturator of the uterus is formed with the help of an implant. For this, the anastomotic zone from the lower segment of the uterus to the upper 1/3 of the vagina is covered from the outside with a mesh implant tape, woven in the form of a stocking from superelastic nickel-titanium threads with a diameter of 10-15 microns with a mesh size of 2-4 mm, an implant width of 35-40 mm and a length of 80-90 mm, followed by fixation with separate seams around the perimeter.
