成果简介NOVELTY - Invention relates to medicine, namely to endoscopic surgery. Percutaneous transhepatic external drainage of the bile ducts is performed. A lithotriptor probe is inserted along the drain and antegrade electropulse lithotripsy is performed under fluoroscopic control until a lumen appears sufficient to insert a conductor. Then, a conductor is inserted through the appeared lumen of the common bile duct and lowered into the duodenum, after which the large duodenal papilla is cannulated along the lowered conductor and endoscopic papillosphincterotomy is performed on the conductor. Then, endoscopic balloon dilatation of the lumen of the intramural part of the choledochus is carried out along the guidewire and a ROS catheter is inserted with its subsequent fixation. A transnasal gastroscope is inserted through the ROS catheter into the lumen of the choledochus, and the balloon is inflated antegradely in the bifurcation zone of the lobar bile ducts. Retrograde endoscopic electropulse lithotripsy is performed, followed by removal of fragments with a Dormia basket and sanitation of the common bile duct. USE - Medicine.
