成果简介NOVELTY - Predicting the decrease in estimated glomerular filtration rate (eGFR) in 12 months after parathyroidectomy (PTE) in patients with primary hyperparathyroidism (PHPT), comprises determining a set of diagnostically significant indicators i.e. gender; age at the time of surgery [years]; body mass index [kg/m2]; laboratory parameters before PTE-parathyroid hormone [pg/ml], ionized calcium [mmol/l], phosphorus [mmol/l], alkaline phosphatase [U/l], total cholesterol [mmol/l], urea [mmol/l], eGFR [ml/min/1.73m2]; instrumental parameters before PTE- bone mineral density SD, diastolic blood pressure [mm Hg.Art.]; anamnesis data before PTE-taking angiotensin-converting enzyme inhibitors for ≥ 6 months before surgery [yes/no], taking angiotensin II receptor blockers for ≥ 6 months before surgery [yes/no], presence of nephrolithiasis [yes/no], duration of nephrolithiasis [months]; and drug therapy parameters before PTE-preoperative administration of at least 1 dose of cinacalcet. USE - The method is useful for predicting decrease in estimated glomerular filtration rate (eGFR) in 12 months after PTE in patients with PHPT. ADVANTAGE - The method can predict decrease in estimated glomerular filtration rate (eGFR). DETAILED DESCRIPTION - Predicting the decrease in estimated glomerular filtration rate (eGFR) in 12 months after parathyroidectomy (PTE) in patients with primary hyperparathyroidism (PHPT), comprises determining a set of diagnostically significant indicators i.e. gender; age at the time of surgery [years]; body mass index [kg/m2]; laboratory parameters before parathyroid extract (PTE)-parathyroid hormone [pg/ml], ionized calcium [mmol/l], phosphorus [mmol/l], alkaline phosphatase [U/l], total cholesterol [mmol/l], urea [mmol/l], eGFR [ml/min/1.73m2]; instrumental parameters before PTE- bone mineral density SD, diastolic blood pressure [mm Hg.Art.]; anamnesis data before PTE-taking angiotensin-converting enzyme inhibitors for ≥ 6 months before surgery [yes/no], taking angiotensin II receptor blockers for ≥ 6 months before surgery [yes/no], presence of nephrolithiasis [yes/no], duration of nephrolithiasis [months]; drug therapy parameters before PTE-preoperative administration of at least 1 dose of cinacalcet not more than 2 days before surgery [yes/no], preoperative administration of cholecalciferol more than 1 week before surgery [yes/no]; anamnesis data after PTE-postoperative hypocalcemia [yes/no]; histological diagnosis based on examination of surgical material of the parathyroid gland (PTG)-adenoma or hyperplasia of the PTG [yes/no]; parameters of drug therapy after PTE - dose of alfacalcidol [mcg/day], dose of calcium preparations [mg/day], dose of cholecalciferol [IU/week], after which the measured parameters are processed using at least one classification model trained to determine the decrease in eGFR 12 months after the PTE. A random forest type model is used as the classification models.
