| 成果简介 | NOVELTY - Method for predicting infertility in women with endometrioid ovarian cyst involves recruiting patients using a continuous sample, collecting anamnesis from all of them, performing a gynecological examination, analyzing somatic pathology, determining 6-catechol-O-methyl transferase (COMT) in morning portion of urine using enzyme immunoassay method to reflect level of melatonin in the blood, studying areas of nucleolar organizers in endometrioid foci of the ovaries, counting the number of nucleoli, intranucleolar inclusions and extranucleolar inclusions, performing imposition of pneumoperitoneum, examining pelvic organs, assessing the stage of ovarian endometriosis, performing statistical processing of data, determining the activity of the nucleolar organizer regions in the epithelial cells of endometrioid ovarian formations, and performing a correlation analysis of the melatonin level and the degree of activity of ribosomal protein synthesis. USE - The method is useful for predicting infertility in women with endometrioid ovarian cyst. DETAILED DESCRIPTION - Method for predicting infertility in women with endometrioid ovarian cyst involves recruiting patients using a continuous sample, collecting anamnesis from all of them, performing a gynecological examination, analyzing somatic pathology, therapeutic and diagnostic laparoscopy, determining 6-catechol-O-methyl transferase (COMT) in morning portion of urine using enzyme immunoassay method to reflect level of melatonin in the blood, studying areas of nucleolar organizers in endometrioid foci of the ovaries and identifying their activity for which they stain tissue sections with a 50% colloidal solution of silver nitrate according to method of J. Crocker and P. Nar, in the stained preparations they visualize nucleoli, inclusions in them in the form of intranuclear and extranuclear argentaffin protein granules associated with nucleolar transcription zones, assessing their activity at a magnification of 1000 with oil immersion, using a microscope Nikon E-200, in the epithelial cells of endometrioid cysts, counting the number of nucleoli, intranucleolar inclusions and extranucleolar inclusions, determining content of nucleoli and argentaffin inclusions in 50 epithelial cells of endometrioid ovarian cysts in each case, expressing them in absolute values per 1 cell, while the representation of cells of types 1 and 2 is expressed as a percentage of the total number of studied cells, performing laparoscopy using Karl Storz equipment, performing imposition of pneumoperitoneum, examining the pelvic organs, the presence and severity of the adhesive process, noting localization and number of foci of endometriosis, assessing the stage of ovarian endometriosis based on classification of L.V. Adamyan, evaluating and identifying patients, dividing them into groups depending on their condition, performing statistical processing of data, determining the activity of the nucleolar organizer regions in the epithelial cells of endometrioid ovarian formations at different stages of endometriosis, performing a correlation analysis of the melatonin level and the degree of activity of ribosomal protein synthesis in patients with different stages of ovarian endometriosis and comparing the stages by the number of intranuclear (INC) argentaffin inclusions inside the nucleoli or diffusely stained nucleoli characterizing low activity of ribosomal synthesis (type 1 cells-INC), and extranuclear (ENC) argentaffin inclusions characterizing high activity of ribosomal synthesis (type 2 cells-ENC), the values of the melatonin level, the type of its connection with INC and INC+ENC to identify their connection with infertility, additionally determining indicators in fibroblasts and histiocytes of the connective tissue of the endometrioid cyst capsule when studying the activity of nucleolar organizer regions in epithelial cells, assessing the condition of patients, dividing them into 2 groups depending on the presence or absence of infertility, performing an additional comparison of stages when conducting a correlation analysis within each of the stages in the presence and absence of infertility, performing a logistic stepwise regression analysis to determine the most important indicators of the activity of nucleolar organizer regions and the level of melatonin and calculating coefficients for statistically significant predictors, allowing an equation to-be drawn up for calculating probability P of infertility development using specific equation involving predetermined parameters such as x1-x7, where x1 is indicator of the number of INC inclusions in epithelial cells, x2 is indicator of the melatonin level, x3 is indicator of the number of inclusions in fibroblasts, x4 is indicator of the number of INC and/or ENC inclusions in fibroblasts, x5 is indicator of the number of nucleoli in histiocytes, x6 is indicator of the number of inclusions in histiocytes and x7 is the indicator of the number of INC and/or ENC inclusions in histiocytes, preferably with a value of P≥ 0.4 and when identifying patients with a reliability of at least 66% of the presence of infertility in them or with a reliability of no more than 62% of the absence of infertility in them, in the first case, the development of infertility in them is predicted, and in the second - the possibility of pregnancy. |
